Job Summary: The CCFT Bachelor's Level Case Manager serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for SPMI client pop! ulation. This person will provide primary case-management for an assigned group of clients, including on-going assessment, rehabilitation and support services. Must have a strong commitment to the SPMI population where the goal is to help the client and family maintain within their natural environment, home, school, and community, and to learn to access appropriate support services.

This individual will also be responsible for:
Act a liaison with community agencies and families to advocate for the rights and preferences of clients and to facilitate the treatment process.

Provides advocacy, linkage, and referral services

Maintains appropriate chart records

Provides mental health assessments, evaluations, and consultative services

Facilitation of groups

Completes all documentation in a timely manner

Participates in treatment team meetings with a psychiatrist

Participates in IEP and other school! related meetings as need or as requested by families

! Interfaces professionally with other agencies involved in the ecology of the family

Provides therapeutic support and crisis intervention, including on-call crisis services 24/7

Upholds center policy and procedures, SSOC, and CARF standards

Knowledge/education in RE-ED philosophy

Perform other duties as assigned

The services are community based with a strong family component (80% of the contacts are family focused). The individual will provide direct clinical services to individuals, groups, and families to facilitate:

Family/client knowledge and use of behavioral techniques to manage symptoms not relieved with medication,
Family/client knowledge and understanding of importance of consistent medication compliance in the treatment process,
Family/client personal growth and development,
Family/client skills and abilities to cope with internal/external stressors, and
Family/client access to natural and commu! nity social support resources.

Family/client stabilization services

In home family therapy

Specific parenting skills training

Family/client Education

Planned family respite

J ob Requirements:
Experience: Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental retardation. One year experience preferred. Experience working with children is preferred although applicants may have other skills and experiences that could accommodate this position.

This person will either posses experience or training in the Re-Education Philosophy or will acquire such training in the course of employment.

Physical: Ability to effectively and ethically counsel children and families. Ability to present professionally and work within a team format to plan! , implement, and evaluate successful interventions. Ability to effectiv! ely run and process groups with children and families. Ability to work within a team format to meet positive goals for children and interface with other agencies involved in the ecology of the family. Must be capable of assisting in utilizing non-violent methods of crisis intervention including therapeutic holding. Must have mental ability to exercise sound judgment under pressure. Ability to exercise effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations. Must also have the ability to communicate effectively and possess good time management and organizational skills. Demonstrated ability to abide by professional/ethical code of conduct and to establish supportive and respectful relationships with clients.

Hearing of normal and soft tones. Close eye work. Valid driver's license. Lifting up to 50lbs. Frequent sitting, standing! , walking, bending, stooping, and reaching.

HRMC is an EOE. HRMC conducts Background Checks, Drivers License Record, Degree Verification, and Drug Screens at hire. Employment is contingent upon clean drug screen, background check and driving record. Additionally, certain programs are subject to TB Screening and/or Testing.

Bachelor's degree in a health-related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPNs must have fifteen (15) semester hours of behavioral health and one (1) year of behavioral health care expectations. Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of course work in behavioral health and at least one (1) year of work experience in the behavioral health setting. This position also requires that the employee has an F Endorsement on his/her Driver License to be available to transp! ort individuals as needed.
