Detailed specification about this occupation opportunity please read the description below. RHD Respite is seeking a Medical Case Manager. This position is responsible for ensuring that all medical services are operated in compliance with RHD and DIDD regulatory requiremen! ts. Duties include overseeing all medical service delivery and the systemic processes designed to monitor and maintain these services. This position will also provide on-site training regarding medical oversight and staff development. Will be required to work conjunctively with RHDs contracted RN in ensuring that all medical management issues are addressed and adherence to regulatory standard is maintained. ESSENTIAL JOB FUNTIONS: I. Program Responsibilities: Monitor each service recipients medical and psychological appointments to ensure that necessary follow-up care is completed and documentation is collected and filed. Monitor monthly Medication Administration Records to ensure that staff are properly recoding the administration of prescribed medications as well as properly transcribing medical orders to the MAR sheet. Maintain the Medical-Matrix database developed to track all required medical and psychiatric assessments and appointments. Provide on-site training and as! sistance to staff regarding the completion of the required med! ical documentation. Review and maintain service recipient medical charts (Home and Office).II. Outreach Attend designated interagency meetings Establish and maintain contact with treating physicians and psychiatrists Act as liaison to treating physicians and psychiatrists Provide training presentations upon request Oversee Medical Management to assure Continuous Quality Improvement Provide medical oversight to site-supervisors Participate in program plan reviews Participate in development of staff training Monitor the overall compliance of medical management requirements and accreditation standards of DIDD and RHD. Support the implementation and maintenance of the RHD Values Philosophy model. III. Mentoring and Training Responsibilities Other Responsibilities: Need to be available (Via Cell Phone) to Unit Director, Assistant Director, or Site-Supervisors during emergency As indicated to insure continuity of care for the safety all service recipients. Establish and maintain ! open communication with staff, as well as parents and/or guardians of individuals receiving services. Promote a positive working relationship through interaction with community and agency organizations, as well as state and local entities Assure that all files and individual records are kept in a complete and organized manner.
Current LPN license preferred. A minimum of five years experience with a medical service or home health provider agency. Familiarity with Division of Intellectual and Developmental Disabilities ( DIDD) requirements as they pertain to compliance for provider agencies.
