With the growth of our offices across the state, Mental Health Cooperative is actively recruiting a SOAR Specialist to help individuals who have disabilities navigate through the application process for SSI or SSDI benefits. In addition to assisting the application process, the Soar Specialist works street outreach to identify candidates for these programs and assess their needs.

Position Summary: ! Identify individuals who have disabilities and who are likely candidates to apply for SSI or SSDI benefits, and helping them navigate through the application process. Responsible for providing street outreach to the homeless, assessing their needs.
Position responsibilities:
1. Provide street outreach to homeless individuals, assessing their needs, and identifying those who have a disabling condition and who may be eligible for SSI or SSDI benefits.
2. Establish rapport with homeless individuals and other agency referrals, and gather information in order to assess eligibility for SSI/SSDI grant services.
3. Complete appropriate referrals for needed services and treatment and link individuals to resources.
4. Be responsible for completing screenings on potential applicants.
5. Complete screening/intake packet on individuals identified as potentially appropriate for program.
6. Determine whether the individual meets the requirements for! inclusion into the program and document appropriately.
7. Responsible for collecting all data necessary for completion of the SSI/SSDI application process.
8. Collect information from referral source, screening, application, functional interview and summary.
9. Submit request for treatment records from past and current providers with appropriate release of information from client.
10. Schedule independent psychiatric evaluations when appropriate.
11. Track receipt of all information and ensure that program timelines are being adhered to.
12. Responsible for compiling all clinical data into a comprehensive medical summary report that addresses the history of the disabling condition and the functional limitations cause by the disability.
13. Complete a summary report on all applicants that includes description of individualâs behavior during interviews, personal and social history, physical health history, psychiatric history, and functional lim! itations.
14. Ensure that medical summary report is co-signed by a ! Licensed Psychiatrist or Psychologist in order for the summary to be submitted as medical evidence to SSA
15. Be responsible for submitting comprehensive medical evidence to SSA/DDS.
16. Ensure that all relevant medical evidence is submitted to SSA/DDS within the timeframes outlined by the program.
17. Track progress of application at SSA and DDS and provide additional information if requested.
18. Inform the client once the claim is approved or denied and assist as needed.
19. Responsible for conducting street outreach to homeless individuals in Davidson County.
20. Insure timely and appropriate documentation as required by Agency including: notes, activity logs, PATH surveys, etc.
21. Attend Soar Network meetings.
22. Responsible for notifying staff of pertinent changes in SSA application processes.
23. Meet regularly with Program Manager of Specialty Teams for supervision.
