Psychiatric Mental Health Nurse Practioner

Location: Harriman Tennessee

Under the supervision of the Chief of Staff, Mental Health Services and/or a local area Psychiatrist, will provide specialized mental health care services in an outpatient, community based outpatient clinic setting.

Responsibilities:
Evaluates and manages psychiatric disorders.
Conducts in-depth interviews with patients; constructs comprehensive medical histories; performs initial and periodic physical examinations.
Coordinates appropriate follow-up care, referrals, and case management. Performs crisis interventions and medication management.
! Develops and implements long and short range treatment plans.
Consults and coordinates care with other medical and psychologist staff and outside agencies.
Serves as a resource in evaluation and management of psychiatric disorders for primary care staff.
Examines patient for symptoms of organic, congenital, or acquired disorders and determines nature and extent, performs comprehensive examinations, referring to diagnostic tests and utilizes medical instruments and equipment when appropriate; documents all medical information into the patient’s electronics medical record utilizing the VA’s VISTA/CPRS electronic patient record system.
Maintains confidentiality of patient data and information.
Orders and performs diagnostic tests and therapeutic procedures and interprets test results for deviations from normal and provides follow up care.
Refers patient to the VA medical specialist when indicated.
Develops and implements patient management plans! , and assists in provision of continuity of care through the PCMM (Primary Care Management Module)
Evaluates patients records from medical providers outside the VA and works with these patients utilizing rules set forth by the VA for co-managed care.
Responsible for participating in Quality Assurance, Performance Improvement activities and staff meetings.
Completes any and all clinical reminders “due” at the time of each patient visit.

Qualifications:
Masters Degree (Nursing or related field) required.
Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioner, including preceptorship
Board Certifed as a Psychiatric Nurse Practitioner by an appropriate/applicable professional organization (i.e., ANCC/AANP)
Current valid nursing license in the state in which the clinic is located.
At lease one year experience within the last 4 years as a psychiatric nurse practitioner in a relat! ed primary care or ambulatory care setting.
Current certificate in ! Basic Cardiac Life Support (must be renewed annually/as required)
Skilled in management of health-illness needed in primary health care. Knowledge of and skills in conducting a health history and performing a physical examination. Proven ability to distinguish between normal and abnormal patient assessment findings.
Skilled in interpretation of laboratory data. Knowledge and skill in assessment and treatment of common acute illnesses and injuries. Knowledge of psychiatric principles, practices, and process gained through extended clinical study and experience.
Knowledge and experience in managing psychniatric medications.
Knowledge of psychiatric models of care with emphasis on interdisciplinary team approach.
Knowledge and skill in assessment and treatment of common acute illnesses and injuries.
Skilled in interviewing and counseling
Knowledge of community agencies most frequently used in mental health care.
Ability to serve a large volume o! f patients rapidly and effectively on a walk-in or appointment basis, within set time limits.
