Community Support Specialist - Older Adult

Description: Centerstone is currently interviewing Community Support Specialist - Older Adult right now, this job will be reside in Tennessee. Our Community Support Specialists provide linkage, referral, and advocacy services to a caseload of adults aged 50 and above with mental illness, including substance ! abuse. Job requires home visits (mileage reimbursed) and documentation of all work with consumers. Training provided during first 90 days of employment. The Older Adult Specialty treatment team includes psychiatrists, nurses, nurse practitioners, therapists and case managers who coordinate care for older adults. We create and deliver individualized treatment plans designed to meet the goals of each client. We also educate older adults and family members, empowering them to play an important role in the recovery process.
Requirements:
Must be able to work within a team based approach. Bachelor’s degree in: (1) social work, psychology, or sociology (2) criminal justice or education with at least 15 semester hours of psychology, social work, counseling or other behavioral health related classes and with at least one year of post degree related work experience. Must have a Tennessee driver’s license, reliable transportation, and liability insurance.
