Description: Centerstone is at present looking to employ Therapist/LCSW - Older Adult Services right now, this job will be placed in Tennessee. Further informations about this job opportunity please give attention to these descriptions. Position includes providing direct individual and group therapy services to older adult mental health consumers. Other duties include performing initial assessments, maintaining treatment plans and completing all clinical documentation. No weekends, holidays, or after hours on-call required. Position may include limited home and community based services.
The Older Adult Specialty treatment team includes psychiatrists, nurses, nurse practitioners, therapists and case managers who coordinate care for older adults. We ! create and deliver individualized treatment plans designed to ! meet the goals of each client. We also educate older adults and family members, empowering them to play an important role in the process.
