Duties:
Incumbent will serve as an dependent practitioner under their Scope! of Practice in the setting requiring the highest level of pharmacist clinical privileges with minimal supervision. Principal responsibilities include:
Pharmacotherapy management, provision of drug information services, and the development and supervision of pharmacy programs in mental health areas based on service needs.
Coordinating all clinical pharmacy functions for VA Mental Health programs as assigned and works with other team members in solving all drug-related problems.
Demonstrating expertise in the area of psychiatric pharmacotherapy and the goals and objectives of the Mental Health programs (acute psychiatry, outpatient psychiatry, and RRTP).
Assuring full compliance with all RRTP VA Directives and Requirements.
Attending and participating in Mental Health interdisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters. Performing initial and periodic assessment of patient's medic! ation therapy, monitoring as needed, and making recommendations as necessary.
Identifying patient-specific medication issues, including drug interactions, adverse effects, lack of effectiveness, appropriateness, and compliance problems.
Directly assisting other providers in rational medication selection by providing written and/or verbal consultation.
Collaborating with other Mental Health staff, educating the veterans and care givers regarding the proper use of medications.
Recommending medication education materials for Mental Patient use as required.
Providing mediation information to other Mental Health team members as required.
Supervising pharmacy trainees assigned to the program.
Providing assistance in developing policies as requested.
Acting as the Pharmacy Service representative to the Mental Health and Behavioral Sciences Council.
Participating in developing criteria for drug utilization evaluations, data collection, trending r! eports, and other quality assurance activities as required to ensure appropriate and safe drug utilization.
Assisting in initiation of medication use improvement strategies.
Being responsible for assuring the safety and efficacy of the mediation use process in the area assigned by reporting adverse drug effects, medication errors, and other quality issues.
Serving as liaison to Pharmacy Service for Mental Health services including but not limited to inpatient psychiatry, psychiatric ambulatory care services (PACS), RRTP, and assisting with clozapine issues should they arise.
Reporting directly to and requiring minimal supervision of the Associate Chief of Pharmacy, Clinical and Educational Services.
Assuring that all prescriptions and actions meet proper eligibility criteria on a patient specific basis and that National Formulary and Non-Formulary procedures are followed.
Assuring compliance with VA and other accrediting bodies in terms of meeting al! l medication use standards.
Actively participating in local, state,! and national organizations related to the advancement of the pharmaceutical profession.

The Pharmacist (Clinical Specialist-Mental Health/RRTP) will serve as the Clinical Pharmacy Specialist and Consultant of the Chief of Pharmacy Service for Mental Health Services, and the Residential Rehabilitation Treatment Program (RRTP).

Qualifications & Evaluations

Education :
(1) Graduate of an Accreditati! on Council for Pharmacy Education (ACPE) accredited College or School o! f Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/ .
(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
