About the Memphis VA Medical Center: The VAMC in Memphis, TN consists ! of one main campus located at 1030 Jefferson Avenue, which inc! ludes a 60-bed Spinal Cord Injury Unit, and ten community based outpatient clinics (CBOC). It is a tertiary care facility classified as a Clinical Referral Level I Facility and one of the most complex medical centers in the VA system. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology, as well as extensive education and research programs. Comprehensive primary, secondary, and tertiary health care is provided in areas of medicine, general cardiovascular and neurological surgery, psychiatry, physical medicine and rehabilitation, spinal cord injury, neurology, oncology, dentistry, and geriatrics. Specialized outpatient services are provided through general, specialty, and subspecialty outpatient clinics including a women's health center. Services are available to more than 206,000 Veterans living in a 53-county tri-state area (Tennessee, Arkansas, and Mississippi).

Background and-or Security Investigation required DUTIES:
The Outpatient Clinical Staff Pharmacist is a representative of the Pharmacy Service, for critical and acute care sections of inpatient beds and for ambulatory care areas. This position covers direct and indirect patient care, appropriate, safe and economical medication use and provides pharmaceutical care services, including monitoring, quality assurance/ improvement, staff/patient education and consultation. The incumbent serves in a multi-functional role and is specifically prepared by pharmacy education and experience to have an advanced knowledge of pharmacology, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.

Dispensing: Fills prescriptions by selecting the correct drug and dosage form, reducing the drug from bulk to the quantity prescribed, compounding when necessary, then packaging and labeling the drug in an app! ropriate manner. Exercises care in the dispensing of drugs, especially controlled substances, investigational and cytotoxic drugs, to assure that legal and agency regulations and instructions are followed, that proper and complete recordkeeping is maintained, and that the drug storage areas are stocked, safe, and secure. Reviews the patient medication profile prior to dispensing drugs and medications. Facilitates special formulations, and extemporaneous compounding of routine and special parenteral medications requiring complex preparation and/or dosing procedures.

Clinical: Prevents and identifies drug-induced problems that may be affecting the patient, takes appropriate steps to manage these problems and/or communicates these findings to his/her primary provider (includes drug side effects, allergic reactions, toxicities, duplicate therapy, dosage regimes, and non-compliance concerns). Counsels patients and family members concerning medication directions f! or use, side effects, precautions, and the need to strictly follow the ! regimen. Monitors patient medication profiles and ensures information is correct and up-to-date. Provides consultation to physicians, nurses, and other health professionals on medication management issues. Makes recommendations in the selection of formulary alternative and/or therapeutic interchange medications when those requested are not available or when a more clinically effective or economical approach appears appropriate to patient needs. Serves as a consultant for proper parenteral medication therapy and scientific compounding techniques for physicians, nurses, and other members of the health care team. Conducts drug usage evaluations, and patient drug therapy monitoring in accordance with VAMC Memphis and Pharmacy Service policy and Joint Commission standards. Applies pharmacokinetics, using rate order equations as appropriate to equilibrate serum levels using volumes of distribution and/or renal and hepatic clearances when necessary. Analyzes laboratory test data s! o as to recommend drug dosing adjustments when necessary and minimize interactions. Performs polypharmacy patient medication reviews and relates as indicated recommendations to the patient's primary provider QUALIFICATIONS REQUIRED:

BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Graduate of a degree program in pharmacy from an approved college or university. PHARM.D PREFERRED.

Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e. Puerto Rico) or the District of Columbia.

Proficient in written and spoken English.

GRADE DETERMINATIONS:
Grade and salary determination will be based on the individual's qualifications and will be recommended by a Professional Standards Board.

SPECIALIZED EXPERIENCE FOR GS-12 LEVEL: Requires the! completion of the equivalent of 1 year of creditable experience at the! next lower grade which is directly related to the position to be filled OR board certification recognized by the American Pharmaceutical Association's Board of Pharmaceutical Specialities. In addition, pharmacists in these assignments must demonstrate the following knowledges, skills, abilities and other characteristics:

1. In depth knowledge of a specialized area of clinical pharmacy practice or a specialty area of pharmacy such as quality assurance/utilization review, ADP systems, drug information, etc.

2. Advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.

3. Skill in monitoring and assessing the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters.

4. Knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to health care.

5. Ability to communicate orally ! and in writing with a wide variety of individuals. This would typically include beneficiaries, professionals in other health care disciplines, and health related groups in the community.

6. Knowledge of the standards related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs).

PHYSICAL REQUIREMENTS:
The work requires standing , walking for short periods of time, light carrying and lifting under 15 pounds; bending, reaching and stooping. The work also involves working around machinery with moving parts and working closely with others.
