Position Overview

Provide mental health services to residents in designated nursing homes

Duties and Responsibilities
Abides by all responsibilities as outlined in signed Independent contractor’s contract with Care-now. Maintains professional liability insurance at all times, forwarding to Care-now all current and renewed policies. Obtains referrals from program director, nursing home staff, and v! arious referral sources. Once insurance information is verifie! d, obtains permission from program manager to provide initial assessment, and subsequent treatment services. Explains and obtains authorization and consent from the resident and/or legal guardian to provide services. Develops initial treatment plan and conducts individual, group, and family therapy in relation to treatment plan. Responsible to inform nursing home staff, primary care physician, CareNow psychiatrist, and family members as needed when aware of any remarkable conditions related to residents physical/mental condition or medication. Participate as a member of the multi-disciplinary treatment team at the nursing facility including participation in Individual Care-plan treatment plan meetings when necessary. Provide Case-management services and advocate for resident as requested or needed. Responsible for completing all sections of the progress notes each time the resident is seen. All documentation should be legible and according to program guidelines. Leaves one ! copy of the treatment plan and progress note with the facility, either on the resident’s chart or provided to a designated contact person within the facility within 7 days of service. Forwards copies of treatment plans and progress notes to Care-Now office within 7 days for billing purposes. Participates in meetings/calls with program manager or designated Care-Now staff as needed. Seeks supervisory assistance and direction in order to maintain a high degree of efficiency, accuracy and proficiency. Completes and forwards all any additional documentation (in-service trainings, informed consent forms, care-plan meetings documentation) to Care-Now office within 7 days of service. Responsible for maintaining client caseload information, dates of services, geographical information, Medicaid/Medicare numbers, Diagnosis, legal guardian, Power of Attorney Nursing home staff, and Primary Care physician contact information, and any pertinent information regarding treatment. Ensures! confidentiality in accordance with HIPAA guidelines for each resident ! and that all residents are treated professionally and with dignity. Provides on-going education, in-service trainings, and marketing efforts to nursing home staff to promote referrals. Assume additional responsibilities as necessary in agreement with Independent contract or program management with Care-Now. Abides by professional ethics, laws, and education requirements to maintain licensure in good standing.
Experience and Education Requirements
Current LCSW in Good standing with the state of Georgia. Minimum of 2 years direct care in counseling/psychiatric nursing; working with the mentally challenged. Knowledge of the developmental process, normal and abnormal, from conception through adulthood; knowledge of the principles, techniques, methods, theory of assessment practice in serving residents with mental illness, mental retardation and developmental disability; knowledge of developmental diagnostic issues in mental illness, mental retardation and developmental ! disability, treatment and management of MI/MR/DD older adults. Effective communication skills, emphasizing diplomacy and assertiveness Excellent verbal and written communication Must be well organized and able to multitask Must maintain confidentiality and exhibit a professional demeanor
