As a Clinical Advisor you will collaboratively perform case management services with medical case managers as indicated for members who have both medical and behavioral health issues. Some key areas of responsibility will be:
Perform telephonic case management with members throughout the United States with a focus upon behavior change.
Complete assessments for members involved in Integrated Medical-Behavioral Case Management (IMBH) services.
Participate in case management services in an effective and efficient manner in accordance with LifeSynch Clinical Department and Program Policy and Procedures.
Assess the needs of identified IMBH members and work with providers and medical counterparts to develop an individualized plan of care and provide care coordination assistance to members, guardians and providers.
Utilize Motivational Interviewing and Solution-Oriented approaches in communication with members.
Work collaboratively with medical case managers to provide consultative assistance and participate in joint care plan.
Identify community and atypical resources to provide services adjunct to treatment plan interventions.
Adhere to URAC standards, state and federal regulations and individual licensing boards.
Attend and participate in scheduled clinical case staffing to include Frontline Leader, Peers, Program Psychiatrist (if applicable).
Close cases efficiently and appropriately as indicated by member progress.
Other tasks as assigned.

Qualifications
Role Essentials
Health professional with a minimum of an Associateâs Degree in Nursing or a Masterâs Degree in the behavioral-health field such as social work, psychology, or a related discipline from an accredited university.
Possess a current, valid and unrestricted license in the state of residence. Such licenses include Licensed Master or Licensed Clinical Social Worker (LMSW or LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist (LP) or Registered Nurse (RN).
CCM certification eligible required.
At least one (1) year experience in mental health or substance abuse case management.
Ability to exercise sound clinical judgment and engage in ethical behavior.
Ability to develop and implement personalized treatment plans.
Ability to conceptualize and present clinical cases in concise and clear manner.
Ability to provide training and feedback to medical case managers as needed.
Role Desirables
Certification as a Case Manager (CCM) preferred.
One (1) year in medical case management preferred.
Reporting Relationships
You will report to a Frontline Leader. This area is under the leadership of the SVP & Chief Operating Officer.
